California Service Intake Checklist
When submitting an inquiry or uploading documents for service under CCP § 415.10 (personal delivery), CCP § 415.20 (substituted service), or CCP § 1161 (unlawful detainer notices), please include the issuing Superior Court division, case number, statutory hearing date, and complete recipient address including suite, apartment, or gate code details.
For corporate and LLC service under CCP § 416.10, verify the exact entity name and designated registered agent on file with the California Secretary of State (including CT Corporation System in Glendale or CSC in Sacramento). Upon completion of service, an executed Judicial Council Form POS-010, POS-015, or SUBP-001 / SUBP-010 Proof of Service is delivered in PDF format ready for Superior Court filing.
